Activity Permission Form

Emergency Medical Form

This form does NOT sign your camper up for activities. It simply gives permission, in the event that they wish to participate in that activity while here at camp. This form also serves as the media release so that we can share pictures of your camper from the week on our social media.

This form provides necessary medical information, in the event that there is an emergency, or if we need to administer any medication while your camper is in our care.
